Instructions for filling out order form:
NOTE: Jerseys are being done “ala carte” this year to allow more people to be able to afford them. Names and numbers are an additional charge, as is shipping if you want them mailed as opposed to me dropping them off to you.
Shipping charge is per order, not per shirt.
· Fill out the Upper table with the quantities of each item and the total costs,
· Fill out the lower table with the sizes, and name and number for the back of the jersey if desired.
· Names can only be alpha characters - no special symbols.
· Fill out bottom section with name, email address and shipping address if needed.
· Before selecting a number, e-mail me at chainsaw@cfl.rr.com to make sure your requested number is available.
· Payment can be in the form of Paypal, check or money order.
· Deadline for orders is August 28, 2011. NO LATE ORDERS WILL BE ACCEPTED.
· Checks and Paypal payments must clear prior to order being accepted, please note your real name, nickname and number on your check or in the Paypal notes.
